Canagliflozin, Metformin falls under ANTIDIABETES DRUGS — the group Milk Thistle interacts with.
Clinical research shows that milk thistle extract, alone or along with tree turmeric extract, can lower blood glucose levels and glycated hemoglobin (HbA1c) in patients with type 2 diabetes, including those already taking antidiabetes drugs. Additionally, animal research shows that milk thistle extract increases the metformin maximum plasma concentration and area under the curve and decreases the renal clearance of metformin, due to inhibition of the multi-drug and toxin extrusion protein 1 (MATE1) renal tubular transport protein.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Invokamet pairs two diabetes medicines, canagliflozin and metformin, that already work to lower your blood sugar. Milk thistle can nudge things in two ways here. First, some studies suggest the silymarin in milk thistle may modestly lower blood sugar and HbA1c on its own. On top of that, animal research hints that milk thistle can slow how your kidneys clear metformin (by blocking a transporter called MATE1), letting more metformin build up in your body.
The practical worry is low blood sugar, with signs like shakiness, sweating, dizziness, or confusion. The metformin part of this is mostly lab and animal data, so it is not certain in people. Still, please check with your pharmacist or doctor before combining them, and don't change your dose on your own.
