Why this applies to Deflazacort

Deflazacort falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for deflazacort (Emflaza). Your body activates and clears it using a liver enzyme called CYP3A4. The thinking is that milk thistle might slow that enzyme down a little, which could let more of the active drug linger in your system. In theory, that might nudge up steroid-type side effects like fluid retention, higher blood sugar, mood changes, stomach upset, or trouble sleeping.

That said, I wouldn't lose sleep over this one. The evidence is weak and conflicting, mostly lab and animal data, and careful human studies didn't show milk thistle meaningfully changing how similar CYP3A4 drugs are processed. The risk is rated minor and unlikely. Still, since deflazacort treats a serious condition, just let your pharmacist or doctor know before adding milk thistle.

What Is Deflazacort Used For?

Brand name: Emflaza

Deflazacort treats Duchenne muscular dystrophy (DMD), a rare genetic muscle disease that causes progressive muscle weakness. It is available under several brand names, each with slightly different age approvals.

Emflaza and Jaythari (both tablets and oral suspension) are approved for patients 2 years of age and older. Deflazacort tablets, Deflazacort oral suspension, Kymbee (tablet and oral suspension), and Pyquvi (oral suspension) are approved for patients 5 years of age and older.
