Why this applies to Elagolix Sodium

Elagolix Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your body uses an enzyme called CYP3A4 to help clear elagolix (Orilissa). The idea on file is that milk thistle might slow that enzyme down a little. If it did, a bit more elagolix could linger in your system, which in theory could mean more of its usual effects like hot flashes, headache, mood changes, nausea, or trouble sleeping.

The honest truth, though, is that this is a weak, mostly theoretical concern. Careful human studies found milk thistle did not meaningfully change the levels of similar CYP3A4 medicines, and the research overall is conflicting. So a real problem with Orilissa is unlikely. Still, since this is a guessing game, it's worth a quick chat with your pharmacist or doctor before adding milk thistle, especially since both can affect the liver.

Elagolix Sodium also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist if you take Orilissa (elagolix) alongside milk thistle. Elagolix is partly moved around your body by a transporter called P-glycoprotein, which acts like a doorman controlling how much of the drug gets absorbed. In the test tube, milk thistle can slow that doorman down a bit, and in theory that could let slightly more elagolix into your system. Elagolix is not a prodrug, so more absorption would mean a touch more of its usual effects, things like hot flashes, headache, nausea, or mood changes.

That said, this is a minor, unlikely interaction. Human studies of milk thistle with other P-gp drugs showed no real change, so the worry stays mostly theoretical. Still, it's worth a quick chat with your pharmacist before adding milk thistle.
