Here's the honest scoop on pairing milk thistle with Yaz (which combines ethinyl estradiol and drospirenone). The only concern we have on file is theoretical. In a lab and animal setting, the silymarin in milk thistle seemed to attach to one type of estrogen receptor. The idea is that it could, in theory, compete a little with the estrogen in your pill and slightly nudge its effect.

In real life, this hasn't been shown to weaken birth control or cause noticeable problems in people, so I wouldn't lose sleep over it. Still, since reliable contraception matters, it's worth a quick mention to your pharmacist or doctor before adding milk thistle, just so everyone's on the same page.

P-GLYCOPROTEIN SUBSTRATES

Ethinyl Estradiol, Drospirenone also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

If you take Yaz (a birth control pill that combines ethinyl estradiol and drospirenone), here is the honest scoop on milk thistle. In test-tube studies, milk thistle can mildly block a transporter called P-glycoprotein, which handles some drugs. In theory, that could nudge absorption of certain medicines upward a little. But this is a minor and unlikely concern, and there is no good evidence it meaningfully changes how your pill works.

For most people, taking milk thistle alongside Yaz should not be a problem. Don't count on it to either weaken your protection or cause new side effects. Still, since your pill matters for both pregnancy prevention and hormone balance, it's smart to run any new supplement past your pharmacist or doctor first.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Good news here: the worry about milk thistle and your birth control pill (Yaz) is mostly theoretical. Both the ethinyl estradiol and drospirenone in Yaz are partly broken down by a liver enzyme called CYP3A4, and people wondered if milk thistle might slow that enzyme down. If it did, a little more hormone could linger in your body, which in theory might mean more of the usual side effects like nausea, breast tenderness, or spotting.

In real-world studies, though, milk thistle did not meaningfully change how the body handles CYP3A4 medicines. The evidence pointing to a problem is weak (mostly lab and animal data), so this is rated minor and unlikely. I wouldn't lose sleep over it, but it never hurts to mention any new supplement to your pharmacist or doctor.
