P-GLYCOPROTEIN SUBSTRATES

Fexofenadine, Pseudoephedrine falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version for Allegra-D. The fexofenadine half of this combo is moved around your body by a transporter called P-glycoprotein. In a test tube, milk thistle can slow that transporter down a bit, which in theory could let a little more fexofenadine get absorbed. If that happened, you might notice slightly more of its usual mild effects, like a dry mouth or a bit of drowsiness, though fexofenadine is pretty non-sedating to begin with.
In real life, though, this looks very unlikely to matter. A human study with another P-glycoprotein drug (digoxin) showed no meaningful change. Fexofenadine isn't a prodrug, so it doesn't get flipped around. I wouldn't lose sleep over it, but if you take both regularly, it never hurts to run it by your pharmacist.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Good news here: this is a very mild, mostly theoretical concern when it comes to your Allegra-D. The idea is that milk thistle might slow down a liver enzyme called CYP3A4, which could let certain drugs build up a bit. But the human studies we have don't show milk thistle meaningfully changing how CYP3A4 medicines are cleared, and the worry mostly comes from lab dishes and a handful of cancer-drug reports.
On top of that, fexofenadine isn't really cleared by CYP3A4 to a big degree, and pseudoephedrine is largely passed out in your urine unchanged. So for most people, milk thistle is unlikely to make this combo cause more drowsiness, jitteriness, or raised blood pressure. Still, it never hurts to run it by your pharmacist, especially if you take other medicines.

ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP)

In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Allegra-D. The fexofenadine half relies on transporters called OATPs to get absorbed from your gut. Milk thistle, in lab studies, seems to gently block one of these (OATP-B1). If that happens in real life, a bit less fexofenadine would make it into your bloodstream, so your allergy relief (think runny nose, sneezing, itchy eyes) might not work quite as well. The pseudoephedrine decongestant part isn't really affected by this.
I'll be honest though: this is based on test-tube research, not solid human studies, so it's more of a theoretical concern than something I'd lose sleep over. If you notice your allergy symptoms creeping back while using both, mention it to your pharmacist or doctor.
