Why this applies to Fomoterol

Fomoterol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the short version: this is a very low-concern pairing. The theory is that milk thistle (through its silymarin compounds) might slow down a liver enzyme called CYP2C9, which could let certain drugs build up a bit. But the evidence is genuinely mixed, some lab studies hint at it while better human studies show no real effect.

On top of that, your formoterol is an inhaled medicine that works right in your lungs, so very little of it depends on that enzyme in a way that would matter. Even if milk thistle nudged your levels slightly, you'd be watching for the usual formoterol effects like a faster heartbeat, jitteriness, or shakiness. Realistically this combination is unlikely to cause trouble, but it never hurts to mention both to your pharmacist.

What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
