Why this applies to Istradefylline

Istradefylline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Your body clears istradefylline (Nourianz) mainly through a liver enzyme called CYP3A4. The thinking here is that milk thistle might slow that enzyme a bit. If it did, istradefylline could build up slightly and you might notice more of its usual side effects, things like nausea, trouble sleeping, dizziness, or in some cases more involuntary movements (dyskinesia).

The good news is this is a minor, unlikely concern. The evidence is weak and conflicting, and human studies of milk thistle with other CYP3A4 drugs mostly showed no real effect. Istradefylline is not a prodrug, so it does not flip the other way. Still, if you plan to take both regularly, give your pharmacist or doctor a heads-up so they can keep an eye on how you feel.

Nourianz (istradefylline) is used alongside levodopa/carbidopa in adults with Parkinson's disease. Specifically, it is meant to reduce 'off' episodes — periods when levodopa stops working as well and Parkinson's symptoms return or worsen.

It is an add-on therapy only; it is not used alone or as a replacement for levodopa/carbidopa.
