Why this applies to Ramelteon

Ramelteon falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the short version: your sleep aid Ramelteon (Rozerem) is partly cleared by a liver enzyme called CYP2C9, and milk thistle might slow that enzyme down a little. If it did, a bit more Ramelteon could linger in your body, which in theory could make you feel groggier or sleepier than usual the next morning.

I want to be straight with you though: the evidence here is shaky and conflicting. Some lab studies hint at this effect, but better human research suggests milk thistle doesn't meaningfully block CYP2C9. We rate this as minor and unlikely. So I wouldn't lose sleep over it. Just mention to your pharmacist or doctor that you take both, and let them know if you notice extra daytime drowsiness.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your body uses an enzyme called CYP3A4 to break down ramelteon (Rozerem), and there's a theory that milk thistle's silymarin could slow that enzyme down. If that happened, more ramelteon could hang around, which might make you feel groggier, more drowsy the next morning, or a bit dizzy. Ramelteon isn't a prodrug, so it doesn't need that enzyme to start working.

The good news is the evidence here is weak. Most careful human studies found milk thistle did not meaningfully change how the body clears CYP3A4 medicines. This concern comes mostly from lab dishes and a few scattered case reports. For an occasional sleep aid like ramelteon, this is unlikely to be a real problem, but it's always worth a quick check with your pharmacist or doctor.

Ramelteon tablets — sold as the brand Rozerem and as generic ramelteon — are used to treat insomnia when the main problem is trouble falling asleep at the start of the night. It is not indicated for difficulty staying asleep.

Clinical studies supporting its effectiveness ran up to six months in both adults and elderly patients.
