Why this applies to Sunitinib

Sunitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your body uses an enzyme called CYP3A4 to clear sunitinib (Sutent) out of your system. The thinking is that milk thistle might slow that enzyme down a little, which could let sunitinib build up and make its side effects (things like fatigue, high blood pressure, mouth sores, diarrhea, or hand-foot skin reactions) more noticeable. Sunitinib works as-is, so it isn't a prodrug, more in your body would mean more effect and more side effects.

But I want to keep this in perspective. The evidence here is weak and mixed, mostly lab studies, and human studies didn't show much real effect. A handful of case reports raised flags with cancer drugs, so since sunitinib is a serious medicine, please run this past your oncologist or pharmacist before adding milk thistle.

What Is Sunitinib Used For?

Brand name: Sutent

Sunitinib malate capsules and Sutent capsules are both approved to treat adult patients with gastrointestinal stromal tumor (GIST) after the cancer has progressed on or could not be tolerated with imatinib mesylate. Both are also used for advanced renal cell carcinoma (kidney cancer) and as adjuvant (preventive) treatment after surgery for adults at high risk of kidney cancer coming back. Additionally, both treat progressive, well-differentiated pancreatic neuroendocrine tumors (pNET) that cannot be surgically removed or have spread.
