Lansoprazole, Amoxicillin, Clarithromycin falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Prevpac is a short course (usually about two weeks) of three medicines (lansoprazole, amoxicillin, and clarithromycin) used to clear an H. pylori stomach infection. The concern here isn't that cinnamon changes how these drugs work. It's that Padang cassia naturally contains coumarin, which in large amounts can stress the liver, and clarithromycin in particular can occasionally be hard on the liver too. Stacking two things that may bother the liver is the worry.

To be clear, this is mostly theoretical, based on how coumarin behaves at high doses. Sprinkling cinnamon on your food is not a problem. The caution applies to concentrated cinnamon supplements, especially if you have liver issues. If you notice yellowing skin or eyes, dark urine, or belly pain, call your doctor. Always check with your pharmacist before adding a supplement.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Lansoprazole, Amoxicillin, Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Prevpac is the combo pack you take to clear an H. pylori stomach infection: it bundles lansoprazole (an acid blocker), amoxicillin, and clarithromycin (two antibiotics). The clarithromycin piece is the one that leans on the liver enzyme CYP3A4. Lab tests hint that a concentrated extract of Padang cassia may slow that enzyme down, so in theory clarithromycin could build up a bit and cause more of its usual nuisances, like stomach upset, nausea, a metallic or bitter taste, or loose stools.

Honestly, this is very weak evidence (just test-tube work), and the cinnamon you sprinkle on food won't matter. My bigger caution is the coumarin in cassia supplements, which can stress the liver. If you're thinking about a concentrated cinnamon capsule during treatment, run it by your pharmacist first.
