Why this applies to Pomalidomide

Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP1A2 substrates may increase the effects of CYP1A2 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP1A2 enzyme activity.

Here's the simple version: your body uses a liver enzyme called CYP1A2 to help break down pomalidomide. In a test-tube study, a concentrated rooibos extract slowed that same enzyme down. In theory, if that slowdown happened in people, pomalidomide could clear a little more slowly and build up, which might mean stronger effects and more side effects, things like low blood counts, fatigue, infections, or a higher risk of blood clots.

That said, this is only lab evidence, not something proven in real patients. A normal cup or two of rooibos tea is very unlikely to cause trouble. The bigger question is concentrated rooibos extracts. Since pomalidomide is a serious cancer medicine, please run any rooibos supplement past your oncology team or pharmacist first.

Your body relies on a liver enzyme called CYP3A4 to help break down pomalidomide (Pomalyst). In test-tube studies, a concentrated rooibos extract seemed to slow that enzyme down. If that happened in real life, pomalidomide could build up a bit and hit harder, which might mean more of its usual side effects like low blood counts, fatigue, infections, or a higher clot risk. Since this is a cancer drug with a narrow safety window, even small shifts are worth taking seriously.

The good news: this is only lab evidence, not proven in people, and a regular cup of rooibos tea is very different from a strong supplement extract. Still, because pomalidomide is so important to get right, please run any rooibos tea or extract past your oncology team or pharmacist first.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
