Rooibos and Omeprazole

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Rooibos interacts with whole categories of medication. Omeprazole falls into 3 of those categories, so it can interact with Rooibos in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 3

Why this applies to Omeprazole

Omeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP2C19 substrates may increase the effects of CYP2C19 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP2C19 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body relies on a liver enzyme called CYP2C19 to break down omeprazole. Lab studies suggest that concentrated rooibos extract may slow that enzyme down. If that happens in real life, omeprazole could linger a bit longer and its effect could be slightly stronger. With a stomach acid medicine, that's usually not dangerous, and might just mean a touch more of the typical side effects like headache, stomach upset, gas, or loose stools.

The reassuring part: this is only seen in test-tube research, not in people, so we really don't know if it matters in everyday life. A normal cup or two of rooibos tea is very unlikely to cause any trouble. If you're using concentrated rooibos supplements, just mention it to your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 3

Omeprazole also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, taking rooibos with CYP2C9 substrates may increase the effects of CYP2C9 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2C9 enzyme activity.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea: your body uses a liver enzyme called CYP2C9 to help clear omeprazole. Lab tests suggest rooibos extract may slow this enzyme down a bit. If that happened in real life, omeprazole could linger a little longer in your system, which might slightly bump up its effects or side effects like headache, nausea, or stomach upset. Omeprazole is not a prodrug, so more drug in the body simply means a stronger, longer effect.

The honest truth is this evidence comes only from test-tube studies, not from people, so I wouldn't lose sleep over it. A regular cup or two of rooibos tea is very unlikely to cause trouble. If you're using concentrated rooibos extracts, just check with your pharmacist first.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Omeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body uses a liver enzyme called CYP3A4 to help break down omeprazole. Lab studies suggest rooibos might slow that enzyme down a bit. If that happened in real life, omeprazole could hang around a little longer and its effects could be slightly stronger. With omeprazole, that might mean a touch more of the usual side effects, like headache, stomach upset, gas, or diarrhea.

The reassuring part: this is only theoretical, based on test-tube research, not on people drinking rooibos. A normal cup or two of rooibos tea is very unlikely to cause a problem. If you're using a concentrated rooibos extract or take omeprazole every day, just give your pharmacist or doctor a quick heads-up.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Omeprazole page

“Omeprazole (Prilosec, Zegerid, Losec) is an acid reducer for heartburn, reflux, and ulcers, and because it sits on pharmacy shelves next to the vitamins, people tend to assume it mixes safely with anything. This list argues otherwise, and it warrants genuine care.”

About Rooibos

What is Rooibos?

Rooibos
  • Caffeine-free beverage
  • Antioxidant support
  • Heart health
  • Blood sugar support
  • Digestive upset
  • General wellness

Rooibos is a caffeine-free herbal tea from South Africa that is rich in antioxidants and generally considered safe as a beverage. While lab and early studies suggest possible benefits for heart health, blood sugar, and inflammation, strong human evidence is lacking for most medical uses. Enjoy it as a tea, but talk to your pharmacist or doctor before using concentrated extracts for health reasons.

About Omeprazole

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.

In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
