Acetaminophen, Aspirin falls under ASPIRIN — the group Tianeptine interacts with.

Chronic use of aspirin might increase the levels and clinical effects of tianeptine.
Aspirin is converted to salicylic acid in the body. In vitro research shows that salicylic acid binds to the same protein sites as tianeptine, displacing protein-bound tianeptine and leading to higher levels of tianeptine in the blood.

Here's the gist with Gemnisyn (which combines acetaminophen and aspirin) and tianeptine. The aspirin part turns into salicylic acid in your body, and lab studies suggest salicylic acid can crowd tianeptine off the proteins that normally carry it in your blood. When tianeptine gets bumped loose like that, more of it floats around free and active, so its effects (and risks) can run higher than expected.

I'll be honest: this signal comes only from test-tube research, not real patients, so it's far from proven. But tianeptine is a bigger worry here than the aspirin. It's not FDA-approved, behaves like an opioid at high doses, and can cause drowsiness, slowed breathing, and dangerous withdrawal. Please talk with a pharmacist or doctor before mixing these.
