Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the thing to watch with this combo. The codeine part of your Aspirin with Codeine is an opioid, so it already slows breathing, brings on drowsiness, and can leave you foggy. Tianeptine, especially in the higher doses found in those gas station products, behaves a lot like an opioid too. Stacking the two means those calming, sedating effects can pile on top of each other.

What that can look like in real life is heavy sedation, slowed or shallow breathing, and a risk of stopping breathing in a serious case. One reported case involved high-dose tianeptine plus a sedative leading to respiratory depression. This concern is based on how these drugs work, not large studies, but please don't combine them without talking to your doctor or pharmacist first.

Chronic use of aspirin might increase the levels and clinical effects of tianeptine.
Aspirin is converted to salicylic acid in the body. In vitro research shows that salicylic acid binds to the same protein sites as tianeptine, displacing protein-bound tianeptine and leading to higher levels of tianeptine in the blood.

First things first: tianeptine isn't FDA-approved here, and the versions sold in gas stations and supplement shops can act like opioids, with real risks of addiction and overdose. I'd urge you to talk with a doctor before going near it. Your medicine, Aspirin with Codeine, already contains an opioid (codeine), so stacking another opioid-like substance on top is the bigger worry I'd want to flag.

On the specific lab finding: the aspirin part of your pill turns into salicylic acid, which may bump tianeptine off its protein binding sites and raise free tianeptine levels. That could mean stronger tianeptine effects like sedation, slowed breathing, or dizziness. This is based only on test-tube research, so it's theoretical, but please run it by your pharmacist before combining these.
