Perampanel falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the thing to keep in mind with Perampanel (Fycompa): it already tends to make people drowsy, dizzy, and unsteady on their feet, and it carries its own warning about sleepiness and even mood changes. Tianeptine, especially at the higher doses found in those 'gas station' products, can act a lot like an opioid and pile on its own sedation. Stack the two together and those calming effects can add up, leaving you more groggy, foggy, or off-balance than you'd expect. In the worst cases, very high tianeptine doses have caused slowed breathing.

This concern is based on how these substances behave rather than solid studies with Perampanel, so it's more of a theoretical caution. Still, please don't use over-the-counter tianeptine, and talk with your pharmacist or doctor first.

Tianeptine is a prescription antidepressant in some countries, but it is NOT approved by the FDA in the United States and is illegally sold in dietary supplements and 'gas station' products. At high doses it acts like an opioid and carries serious risks of addiction, overdose, and dangerous withdrawal. We strongly advise against using over-the-counter tianeptine products and urge anyone considering it to talk with a doctor or pharmacist first.

About Perampanel

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
