Ramelteon falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Ramelteon (Rozerem) is a sleep aid that nudges your melatonin receptors to help you drift off. On its own it's pretty gentle. The concern here is stacking sedation. Tianeptine, especially in the high doses found in those 'gas station' products, can act like an opioid and pile on drowsiness. Put the two together and you may feel more sleepy, foggy, or unsteady than expected, and in serious cases slowed breathing becomes a real worry.

Honestly, my bigger concern is the tianeptine itself, which isn't FDA approved and carries real risks of addiction and overdose. This particular combo is based mostly on how these drugs behave in theory, not strong studies, but please don't mix them without talking to your doctor or pharmacist first.

Ramelteon tablets — sold as the brand Rozerem and as generic ramelteon — are used to treat insomnia when the main problem is trouble falling asleep at the start of the night. It is not indicated for difficulty staying asleep.

Clinical studies supporting its effectiveness ran up to six months in both adults and elderly patients.
