HMS Hemp Extract contains 2 active ingredients: hemp oil and methylsulfonylmethane (MSM). Hemp oil is extracted from hemp seed and is thought to contribute estrogenic and blood pressure-modulating compounds.
MSM is an organic sulfur compound, though we hold no monograph data on its specific roles. The capsules themselves are made from vegetable cellulose and cellulose — the inactive ingredients that hold and deliver the actives.

The evidence we hold for hemp oil's effectiveness is limited. For constipation, atopic dermatitis (eczema), and familial hypercholesterolemia (a genetic cholesterol condition), the data shows insufficient reliable evidence to rate whether it works.
That doesn't mean it's ineffective — it means the research isn't yet strong enough to say one way or the other.
How safe is it?
Well-documented data
Hemp seed foods and oils are generally well tolerated in normal dietary amounts. However, concentrated hemp extracts and supplements like this one are less well studied, so caution is advised.
In larger supplement doses, side effects are uncommon, but rare cases of allergic reactions — including anaphylaxis, hives, facial swelling, and breathing problems — have been reported. There's also a rare case report of elevated liver enzymes and hepatitis in a young child given very high doses of hemp extract for seizures.

Hemp oil also has documented interactions with blood pressure medications. It may add to the effects of antihypertensive drugs and ACE inhibitors (a class used to lower blood pressure), theoretically raising the risk of low blood pressure.
Additionally, there's a theoretical concern with blood thinners and antiplatelet drugs — hemp seed showed antiplatelet activity in animal studies, though human studies at typical supplement doses found no effect. Finally, hemp oil may induce liver enzymes that break down certain medications, potentially reducing the effectiveness of drugs processed through CYP1A2 and CYP3A4 pathways.

Theoretically, hemp might interfere with hormone therapy due to its estrogenic effects.
In an ovariectomized animal model, a diet containing hemp seed 1%, 2%, or 10% resulted in normalized plasma levels of 17-beta-estradiol. The mechanism of action for this effect is unclear.

Theoretically, consuming hemp seed protein isolate with ACE inhibitors might have additive effects and increase the risk of hypotension.
Hemp seed protein hydrolysate has shown ACE inhibitor-like effects in a hypertensive animal model. However, hempseed oil consumption does not seem to reduce blood pressure in humans. Until more is known, monitor blood pressure and potassium levels.
Anticoagulant/Antiplatelet Drugs
Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.
Antihypertensive Drugs
Theoretically, hemp seed protein may have additive effects with antihypertensive drugs.
In a hypertensive animal model, hemp seed protein hydrolysate reduced systolic blood pressure by a mechanism possibly involving the inhibition of renin and angiotensin converting enzyme (ACE) activities. However, there was no effect of hemp seed protein on blood pressure in normotensive animals. Furthermore, hempseed oil consumption does not seem to reduce blood pressure in humans.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, hemp might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that hemp induces CYP1A2 enzymes.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.
