Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Midol Max Strength (which mixes acetaminophen, caffeine, and pyrilamine). Once your liver turns adrafinil into modafinil, that modafinil may gently speed up an enzyme called CYP1A2. The caffeine piece is the one cleared by that enzyme, so in theory your body could break it down a little faster, meaning the caffeine 'pick-me-up' might feel slightly weaker.
I want to be honest: this is theoretical, based on lab studies, not real patient cases, and the effect looks weak. It shouldn't change how the acetaminophen or pyrilamine work. A bigger practical concern is stacking two stimulants, since both adrafinil and caffeine can leave you jittery or wired. Please run this by your pharmacist before combining them.

Here's the gist with Midol Max Strength (acetaminophen, caffeine, and pyrilamine). Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4. Since part of this combo is broken down by that enzyme, in theory it could get cleared a little faster, which might make the pain relief and antihistamine effects feel slightly weaker than usual.
I want to be honest though: this is a theoretical concern based on how the drugs work, not something well documented in real patients, and the effect is considered weak. For occasional menstrual relief it's unlikely to be a big deal. Still, adrafinil is unregulated and hard on the liver, so please run it by your pharmacist or doctor before combining them.

Here's the thing to keep in mind with this combo: that Midol product contains caffeine as one of its three ingredients. Adrafinil turns into modafinil in your body, which is a wakefulness-promoting stimulant. Stack a stimulant on top of caffeine and you can get more of the jittery, overstimulated effects, things like a racing heart, feeling wired or anxious, trouble sleeping, or a touch of an upset stomach.
This is a theoretical concern based on how these substances work, not something proven in studies, so don't panic. The amount of caffeine in a Midol dose is fairly modest. Still, since adrafinil is unregulated and hard on the liver, I'd run it by your pharmacist or doctor before pairing the two.
