Aliskiren falls under ANTIHYPERTENSIVE DRUGS — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind if you're taking aliskiren (Tekturna) for blood pressure and thinking about adrafinil. Your body turns adrafinil into modafinil, a wakefulness drug that has been shown to nudge blood pressure upward in some people. Since aliskiren's whole job is to lower your blood pressure, adrafinil could work against it, possibly leaving your numbers higher than you and your doctor are aiming for.
I want to be honest about the strength of this concern: it's theoretical, based on how these substances behave rather than solid studies pairing them together. Still, if you ever combine them, it would be smart to monitor your blood pressure and loop in your doctor or pharmacist before adding adrafinil at all.

Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Tekturna (aliskiren), your blood pressure pill. When your liver processes adrafinil, it turns it into modafinil. Modafinil can gently nudge a liver enzyme called CYP3A4 to work a little faster. Aliskiren is partly handled by that same enzyme, so in theory it could be cleared a bit quicker and end up slightly weaker.
If that happened, the most likely real-world result would be your blood pressure not staying as low as it should. I want to be honest though: this concern is theoretical, based on how these drugs behave on paper rather than on solid studies, and modafinil is only a weak enzyme booster. Still, before combining them, check in with your doctor or pharmacist, and keep an eye on your home blood pressure readings.
