Here's the thinking behind this combo. When your liver turns adrafinil into modafinil, that modafinil can mildly block an enzyme called CYP2C19, which is one of the pathways that helps clear amitriptyline (the antidepressant half of this combo pill). If that enzyme slows down, amitriptyline levels could creep up, meaning more of its usual effects. With a tricyclic like amitriptyline, that can show up as extra drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat. The perphenazine piece isn't the main concern here.

I'd flag this as theoretical, based on lab data rather than real patient reports, so it may not amount to much. Still, since both drugs affect the brain and heart, please run it by your pharmacist or doctor first.

Here's the gist for your Triavil (which combines amitriptyline, an antidepressant, with perphenazine, an antipsychotic). Both of these are partly cleared by a liver enzyme called CYP1A2. Adrafinil turns into modafinil in your body, and modafinil may gently speed up that enzyme. In theory, that could clear these drugs a little faster and make them work less well, so you might notice your mood symptoms, anxiety, or sleep slipping, or feel like the medicine just isn't doing as much.

I want to be honest: this is a theoretical concern based on lab data, not real patient reports, and the effect on the enzyme is described as weak. Still, since adrafinil is unregulated and hard on the liver, please run it by your pharmacist or doctor before mixing the two.
