This combo product mixes aspirin, caffeine, and hydrocodone. The piece that interacts here is the caffeine, which your liver clears using an enzyme called CYP1A2. Adrafinil (which your body turns into modafinil) may gently speed that enzyme up. In theory, that could clear caffeine a bit faster, so the caffeine portion might feel slightly weaker or wear off sooner.
Honestly, this is a theoretical concern based on lab data, not real patient reports, so I wouldn't lose sleep over it. The aspirin and hydrocodone parts aren't really affected by this. The bigger reason to check in with your pharmacist is that adrafinil is unregulated and can stress the liver, especially alongside acetaminophen-free pain combos like this one.

Here's the gist for this combo pill (aspirin, caffeine, and hydrocodone). When your liver turns adrafinil into modafinil, that modafinil can gently speed up an enzyme called CYP3A4. Hydrocodone leans on that same enzyme as part of how it's processed in the body. So in theory, taking adrafinil alongside this medication could nudge your levels a little lower, meaning you might get less pain relief than you'd expect.
Honestly though, this is a theoretical concern based on how the drugs work, not from strong studies, and modafinil's effect on the enzyme is described as weak. Here's the piece to watch with Damason-P: this product is a combo medicine, and one of its three ingredients is caffeine, which is a mild stimulant. Adrafinil turns into modafinil in your body, and modafinil also has stimulant-like, wakefulness-promoting effects. Stacking the two could add up, so you might feel jittery, anxious, have a racing or pounding heartbeat, or trouble sleeping.
To be fair, this is a theoretical concern based on how these substances work, not on strong studies, so it may not affect everyone. Keep in mind the hydrocodone (an opioid) and aspirin parts of this pill carry their own separate risks.
