Adrafinil and Cariprazine Hydrochloride

Why this applies to Cariprazine Hydrochloride

Cariprazine Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Vraylar (cariprazine). Your liver uses an enzyme called CYP3A4 to process this medication. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that could clear cariprazine a little faster and leave less of it working in your system.

If that happened, you might notice your symptoms creeping back, things like mood instability, agitation, or returning psychotic symptoms. I'll be honest with you though: this concern is theoretical, based on how the drugs work on paper, not on real patient reports, and modafinil is only a weak booster of this enzyme. Still, because cariprazine matters so much for keeping you stable, please loop in your doctor or pharmacist before pairing these.

From our pharmacist’s overview on the Cariprazine Hydrochloride page

“Cariprazine (Vraylar), an antipsychotic prescribed for schizophrenia and bipolar disorder, is a medication I would pair with supplements only carefully, mostly because the liver enzyme that clears it, CYP3A4, is easy to disturb. The great majority of the 145 flagged ingredients either block that enzyme and let cariprazine build up in t…”
