Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the thing to keep in mind with Diovan HCT (the combo of valsartan and hydrochlorothiazide). You're taking it to bring your blood pressure down. Adrafinil turns into modafinil in your body, and modafinil is a mild stimulant that can nudge blood pressure up. So in theory, the two pull in opposite directions, and the adrafinil could chip away at how well your blood pressure pills are working.

I'll be honest, this is a theoretical concern based on how these drugs behave, not strong proof of trouble. Still, if you choose to use adrafinil, it's smart to monitor your blood pressure at home and loop in your doctor or pharmacist. Don't stop or adjust your Diovan HCT on your own. And remember, adrafinil is unregulated and can stress the liver, so it deserves a real conversation first.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C9 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.

Here's the gist for your Diovan HCT (valsartan plus hydrochlorothiazide). Adrafinil turns into modafinil in your body, and lab studies hint that modafinil may slow down a liver enzyme called CYP2C9. Valsartan is partly handled by that enzyme, so in theory it could build up a little and push its blood-pressure-lowering effect harder than expected. That might show up as dizziness, lightheadedness when you stand, or feeling unusually tired.

I want to be honest though: this is theoretical, based on test-tube data, not real patient reports, so it may never amount to anything noticeable. Still, since both can affect how you feel, it's worth a quick chat with your pharmacist or doctor before combining them, and keep an eye on your blood pressure.
