Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the basic idea: lercanidipine (Zanidip) is a calcium channel blocker that you take to bring your blood pressure down. Adrafinil, on the other hand, gets turned by your liver into modafinil, a stimulant that can nudge your blood pressure and heart rate up a little. So the two are working in opposite directions, and the supplement could blunt some of the blood-pressure-lowering benefit you're counting on from lercanidipine.

I'll be honest with you, this is a theoretical concern based mostly on how these drugs behave, not strong head-to-head studies, so for many people the effect would be small. Still, if you ever notice your readings creeping up, headaches, or a faster heartbeat, please check in with your doctor or pharmacist before changing anything.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist with lercanidipine, the blood pressure medicine. Your liver relies on an enzyme called CYP3A4 to break it down. When adrafinil turns into modafinil in your body, that modafinil can mildly speed up CYP3A4. The worry is that this could clear lercanidipine out a little faster, leaving you with a weaker blood pressure lowering effect than usual.

In plain terms, your numbers might creep up if this happens. Keep in mind this is a theoretical concern based on how the drugs work, not something proven in studies, and modafinil is only a weak inducer. Still, since adrafinil is unregulated and hard on the liver, please check with your pharmacist or doctor before combining the two.
