Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the thing to keep in mind if you take losartan (Cozaar) for blood pressure. Adrafinil turns into modafinil in your body, and modafinil can nudge your blood pressure upward. Losartan works the opposite way, lowering blood pressure by relaxing your blood vessels. So adding adrafinil could blunt how well your losartan controls your numbers.

In practice, you might see your blood pressure creep up, or notice less benefit than usual. This is a theoretical concern based on how these drugs work, not a strong, well-proven interaction, so try not to overworry. Still, if you decide to use adrafinil, it's worth checking your blood pressure at home and chatting with your pharmacist or doctor first.

Here's the wrinkle worth knowing: losartan itself is a bit like a prodrug. Your liver uses an enzyme called CYP2C9 to turn losartan into its more powerful active form, which does most of the blood-pressure lowering. Adrafinil (which your body converts to modafinil) may slow that same enzyme down. In theory, that could mean less of the active form gets made, so your losartan might not work quite as well.

If that happened, you could see your blood pressure creep up. That said, this is a theoretical concern based only on lab studies, not real patient reports, so I wouldn't panic. Adrafinil also carries its own liver risks. Please loop in your doctor or pharmacist before combining them, and keep an eye on your blood pressure readings.

Here's the gist for your blood pressure pill. Once your liver turns adrafinil into modafinil, that modafinil can gently speed up a liver enzyme called CYP3A4, which is one of the pathways that helps process losartan. If that enzyme runs faster, your body may clear losartan a little quicker, so it could become slightly less effective.

In real life that might look like your blood pressure creeping up, or less protection than you'd expect. The good news is this is a theoretical concern based on how the drugs work, not strong proof, and the enzyme effect is described as weak. Still, since adrafinil is unregulated and hard on the liver, please check with your pharmacist or doctor before combining them.

What Is Losartan Used For?

Brand name: Cozaar

Losartan treats high blood pressure (hypertension) in adults and children 6 years and older. Keeping blood pressure under control lowers the risk of serious events like stroke and heart attack.

It is also used to reduce the risk of stroke in people who have high blood pressure and left ventricular hypertrophy (an enlarged heart muscle). Note: evidence suggests this stroke-risk benefit may not apply to Black patients. Additionally, losartan treats diabetic nephropathy — kidney damage caused by type 2 diabetes — in people who also have high blood pressure.
