Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the thing to know if you're taking nicardipine for blood pressure or chest pain: your body turns adrafinil into modafinil, and modafinil can nudge your blood pressure upward. That works against what nicardipine is doing, which is relaxing your blood vessels to bring your numbers down. So the worry is a slightly weaker blood pressure effect, meaning your readings could creep up or your angina might feel less controlled.

To be fair, this is a theoretical concern based on how these drugs behave, not strong proof, and any effect is likely small. Still, if you use adrafinil, it's worth checking your blood pressure at home and giving your pharmacist or doctor a heads-up so they can keep an eye on it.

Here's the gist for this pairing. Your body has a liver enzyme called CYP3A4 that helps clear modafinil, the active drug that adrafinil turns into. Nicardipine can mildly block (inhibit) that same enzyme. In theory, that could slow how fast modafinil is broken down, letting it build up a bit and pushing its stimulant effects (and side effects) stronger. That might mean more jitteriness, a racing or pounding heartbeat, trouble sleeping, or higher blood pressure, which is the opposite of what you want when you're treating blood pressure or chest pain.

Honestly, this is a theoretical concern based on how these drugs work, not strong proof. Still, since adrafinil is unregulated and rough on the liver, please check with your pharmacist or doctor before combining them.

Here's the gist for nicardipine. Your liver uses an enzyme called CYP3A4 to clear this blood pressure medicine. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that could clear nicardipine a little faster, leaving slightly less of it working to relax your blood vessels.

If that happened, you might notice your blood pressure creeping up or your chest pain (angina) being less well controlled. I want to be honest though: this is a theoretical concern based on how these drugs work, not something proven in studies, and the effect on the enzyme is described as weak. Still, since adrafinil is unregulated and hard on the liver, please check with your doctor or pharmacist before combining the two.

What Is Nicardipine Used For?

Brand names: Cardene, Cardene SR, Cardene IV

Nicardipine Hydrochloride capsules are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain that comes on with physical effort). The capsules can be used alone or combined with other blood pressure or heart medications.

Intravenous (IV) nicardipine products — including Cardene I.V., Nicardipine Hydrochloride Injection, and Nicardipine Hydrochloride in Sodium Chloride — are used for the short-term treatment of high blood pressure in hospital settings when a patient cannot take oral medication. Once stable, patients are switched to oral therapy.
