Theoretically, adrafinil might increase the levels and clinical effects of CYP2C19 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.

Here's the situation with these two. Your body turns adrafinil into modafinil, and modafinil can put the brakes on a liver enzyme called CYP2C19. That same enzyme is one of the main ones your body uses to break down pantoprazole. So in theory, if that pathway slows down, more pantoprazole could linger in your system than usual.

The good news is pantoprazole is a pretty forgiving medication, and a little extra usually isn't dangerous. At most you might notice more of its typical side effects, like headache, an upset stomach, or diarrhea. Keep in mind this interaction is theoretical, based only on lab studies, so it may never amount to anything you feel. Still, since adrafinil is unregulated and hard on the liver, please run it by your pharmacist or doctor before combining them.

Here's the gist for these two. Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up a liver enzyme called CYP3A4. Your pantoprazole is partly cleared by that same enzyme, so in theory a faster enzyme could clear it a little quicker, leaving slightly less in your system.

What might that look like? Possibly a bit less acid control, meaning heartburn or reflux symptoms could creep back. I want to be honest though: this is a theoretical concern based on how the drugs work, not something well documented in real patients, and modafinil is only a weak inducer. Most people likely won't notice a thing. Still, since adrafinil is unregulated and hard on the liver, please run it by your pharmacist or doctor before combining them.

About Pantoprazole

What Is Pantoprazole Used For?

Brand names: Protonix, Pantoloc, Protonix IV

Pantoprazole delayed-release tablets and oral suspension are used in adults and children 5 years and older to heal erosive esophagitis (EE) — damage to the esophagus caused by stomach acid — and to relieve GERD symptoms short-term (up to 8 weeks). In adults, they are also used to maintain that healing and reduce heartburn relapse, and to treat conditions where the stomach makes too much acid, including Zollinger-Ellison (ZE) Syndrome.

Intravenous (IV) pantoprazole — given by injection or infusion in a clinical setting — is approved in adults for short-term treatment of GERD with a history of erosive esophagitis (up to 10 days) and for pathological hypersecretory conditions including ZE Syndrome. PROTONIX I.V. is also approved for GERD/EE in pediatric patients 3 months and older (up to 7 days). IV pantoprazole is not approved for treating upper gastrointestinal bleeding.
