Adrafinil and Reserpine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Reserpine

Reserpine falls under ANTIHYPERTENSIVE DRUGS — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: reserpine works to lower your blood pressure, while adrafinil turns into modafinil in your body, and modafinil tends to nudge blood pressure up a bit. So in theory, these two push in opposite directions. Adrafinil could blunt some of the blood-pressure-lowering benefit you're getting from reserpine, meaning your numbers might creep higher than you'd like.

I want to be honest about the strength of this, though. It's a theoretical concern based on how these drugs work, not something proven to happen often. The real-world effect seen with modafinil was small. Still, if you're managing high blood pressure, it's worth a quick chat with your pharmacist or doctor before combining them, and keeping an eye on your readings is a smart move.

From our pharmacist’s overview on the Reserpine page

“Reserpine (Serpasil, Sandril) lowers blood pressure, and the 138 supplements and herbs listed here merit real vigilance, four of them rated major. Most pull in one of two directions: many common supplements lower pressure on their own and can add to reserpine's effect, dropping it too far, while a smaller group raises pressure and work…”

About Reserpine

What Is Reserpine Used For?

Brand names: Serpasil, Sandril

  • Reserpine is used to treat high blood pressure. It also is used to treat severe agitation in patients with mental disorders. Reserpine is in a class of medications called rauwolfia alkaloids. It works by slowing the activity of the nervous system, causing the heartbeat to slow and the blood vessels to relax.

  • High blood pressure is a common condition and when not treated, can cause damage to the brain, heart, blood vessels, kidneys and other parts of the body. Damage to these organs may cause heart disease, a heart attack, heart failure, stroke, kidney failure, loss of vision, and other problems. In addition to taking medication, making lifestyle changes will also help to control your blood pressure. These changes include eating a diet that is low in fat and salt, maintaining a healthy weight, exercising at least 30 minutes most days, not smoking, and using alcohol in moderation.
