Spironolactone falls under ANTIHYPERTENSIVE DRUGS — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the gist: you're taking spironolactone to help control your blood pressure (and to manage fluid or potassium issues), while adrafinil turns into modafinil in your body, and modafinil has a mild tendency to nudge blood pressure up. So these two can work against each other. The concern is that adrafinil might blunt how well your spironolactone keeps your numbers down.

To be honest, this is a theoretical concern based on how modafinil behaves, not solid proof, so don't panic. But it's worth knowing. If you choose to use adrafinil, keep an eye on your blood pressure at home and watch for headaches or a racing feeling. Please loop in your doctor or pharmacist before combining them, especially since adrafinil is unregulated and can stress the liver.

About Spironolactone

What Is Spironolactone Used For?

Brand names: Aldactone, Spiroton, Carospir

Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.

Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
