Drug & supplement interaction

Breadfruit + Methadone

Methadone brand name: Dolophine.

Minor Interaction
The bottom line Minor

Breadfruit has a minor interaction with Methadone. Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution.

Breadfruit and Methadone

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Methadone

Methadone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for methadone: your liver uses an enzyme called CYP3A4 to help break it down. Lab and animal studies hint that compounds in breadfruit leaves might mildly slow that enzyme. If that happened in a real person, methadone could clear a little slower and build up, which in theory could mean more sedation, slowed breathing, or constipation.

I want to be honest about the strength here: this is only test-tube and animal data, the rating is minor, and we don't know if eating breadfruit or sipping the leaf tea matters at all in people. Breadfruit as a food is very unlikely to be a problem. Because methadone is a powerful opioid where small changes carry weight, please loop in your pharmacist or doctor before using any concentrated breadfruit leaf products.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

About Breadfruit

What is Breadfruit?

  • Source of nutrition and energy
  • High blood pressure
  • Diabetes and blood sugar support
  • Inflammation and joint discomfort
  • Skin conditions

Breadfruit is a starchy tropical fruit eaten as a staple food, and its leaves are used in traditional medicine for conditions like high blood pressure and diabetes. As a food it is nutritious and generally safe, but the medicinal use of leaf teas or extracts has very little human research to back it up. Talk with your pharmacist or doctor before using breadfruit products for a health condition.

About Methadone

What Is Methadone Used For?

Brand name: Dolophine

Methadone treats two main problems. Some products are used for severe, persistent pain that needs daily, around-the-clock opioid treatment when other options are not adequate. Some products are used for opioid addiction (heroin or other morphine-like drugs), as detoxification or as maintenance treatment along with social and medical services.

Not every product is approved for every use. Methadone tablets, oral solution and oral concentrate (Intensol) list both pain and opioid addiction. Diskets, Methadose Dispersible tablets and the Methadose and Methadone Hydrocloride oral concentrates are labeled only for opioid addiction. Methadone injection is for severe pain, and for short-term treatment of opioid dependence in people who cannot take medicine by mouth, such as hospitalized patients. It is not approved for outpatient opioid dependence treatment. Methadone is not meant to be used as an as-needed pain reliever.
