Mometasone, formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cajeput Oil interacts with.

Evidence from in vitro research suggests that cajeput leaf extract can inhibit cytochrome P450 2D6 (CYP2D6) by approximately 30%. Theoretically, cajeput oil may increase levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the good news on Zenhale (mometasone plus formoterol): this is an inhaler, so very little of it gets into your bloodstream to begin with. The flag on file is about cajeput oil possibly blocking a liver enzyme called CYP2D6 by roughly 30 percent in a lab dish. Of the two ingredients, formoterol isn't a CYP2D6 prodrug, so the worry would just be slightly higher levels rather than a loss of effect.

In real life, this is theoretical and based only on test-tube data. If anything did build up, you might notice a faster heartbeat, jitteriness, or mild shakiness. That's unlikely with normal inhaler use. Still, run any cajeput oil use past your pharmacist or doctor first, and never swallow the oil.
