Why this applies to Cabotegravir, Rilpivirine
Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Cabenuva (the long-acting cabotegravir and rilpivirine injection) and cascara sagrada. Rilpivirine is partly cleared by a liver enzyme called CYP3A4. In lab dishes, cascara sagrada appears to speed that enzyme up a little, which in theory could clear the drug a bit faster and lower its levels. With an HIV medication, lower levels are the worry, since you want the virus stayed fully suppressed.
That said, this is very weak, test-tube-only evidence, and the effect looked far weaker than a strong inducer like rifampin. Honestly, I'd be more concerned about cascara's cramping, dehydration, and electrolyte issues than this interaction. Since Cabenuva keeps your HIV in check, it's worth a quick chat with your pharmacist or doctor before using cascara at all.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Cabotegravir and Rilpivirine Injections Used For?
Brand name: Cabenuva
CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.
The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
