Devil's Claw and Disopyramide Phosphate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Disopyramide Phosphate

Disopyramide Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Devil's Claw interacts with.

Theoretically, devil's claw might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear disopyramide out of your system. In lab dish studies, devil's claw appears to slow that enzyme down. If that happened in a real person, disopyramide could build up a little higher than expected. Since this is a heart rhythm drug with a narrow safety window, even modest extra levels can matter. You might notice things like dry mouth, blurry vision, trouble urinating, constipation, lightheadedness, or in rarer cases, new rhythm changes.

The honest part: this warning comes only from test-tube research, not from people, so it's theoretical. I wouldn't panic, but I would loop in your pharmacist or cardiologist before combining them, given how carefully disopyramide needs to be dosed.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Disopyramide Phosphate page

“Grapefruit is the first thing I would set aside on disopyramide phosphate (Norpace), a medicine that steadies dangerous heart rhythms, because grapefruit juice can push the drug's blood level up. That pattern dominates the page, since 121 of the 169 flagged ingredients act on the same liver enzyme, CYP3A4, raising the drug's level or,…”

About Disopyramide

What Is Disopyramide Used For?

Brand names: Norpace CR, Norpace

Disopyramide treats documented ventricular arrhythmias (dangerous rhythm problems that start in the lower chambers of the heart). An example is sustained ventricular tachycardia (a fast, abnormal heartbeat). It is for rhythm problems that your doctor judges to be life-threatening.

This applies to Norpace and Norpace CR, and to disopyramide phosphate capsules. It is generally not recommended for milder rhythm problems. It should be avoided for asymptomatic ventricular premature contractions (extra beats that cause no symptoms).
