Elranatamab falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the concern with this pairing. Elranatamab (Elrexfio) is a bispecific antibody used for multiple myeloma, and one of its known risks is liver stress, with lab tests sometimes showing raised liver enzymes. Epistane is an oral, 17-alpha-alkylated anabolic steroid that is notoriously hard on the liver. Putting two liver-stressing agents together may add up, so the worry is stacked strain on your liver.

To be honest, this is a theoretical interaction based on how each acts, not proven cases of the two combined. Still, epistane is not a true supplement, it's an illegal designer steroid we'd steer you away from entirely. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please talk with your oncologist or pharmacist before combining anything with Elrexfio.
