HEPATOTOXIC DRUGS
Why this applies to Mirikizumab-mrkz
Mirikizumab-mrkz falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing these two. Epistane is a methylated anabolic steroid that's known to be hard on your liver. Mirikizumab (Omvoh) is an injected antibody used for ulcerative colitis, and while liver problems aren't common with it, elevated liver enzymes have been reported in some people taking it. So the worry is simple: two things that can each stress the liver, used together, might add up.
I want to be honest that this is a theoretical concern based on how these drugs behave, not proven cases of them harming livers together. Even so, Epistane is genuinely liver-toxic and illegal to sell here, so I'd steer clear of it entirely. Please loop in your doctor or pharmacist before mixing anything with Omvoh.

From our pharmacist’s overview on the Mirikizumab-mrkz page
“Mirikizumab (Omvoh) treats inflammatory bowel disease by dialing down part of the immune system, and its interaction list deserves genuine wariness, most of all around the liver. The three most serious entries, Siberian cocklebur plus beer and wine, all raise the odds of liver damage when combined with a medication that can stress the…”

Epistane is a synthetic anabolic steroid (a methylated 'designer steroid'), not a true dietary supplement or herb, even though it has been sold as a 'prohormone.' It is liver-toxic, can disrupt your natural hormones, and is illegal to sell as a supplement in the US under the Designer Anabolic Steroid Control Act. We strongly recommend avoiding it and talking with a doctor or pharmacist.

Brand name: Omvoh
Omvoh (mirikizumab-mrkz) is used to treat moderately to severely active ulcerative colitis in adults — a condition where the large intestine becomes chronically inflamed and ulcerated. It is also used to treat moderately to severely active Crohn's disease in adults, another form of inflammatory bowel disease that can affect any part of the digestive tract.
