Nalidixic Acid falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the concern when you pair these two: both can be tough on your liver. Epistane is a methylated anabolic steroid that is well known to stress liver cells, and nalidixic acid (NegGram), an older antibiotic for urinary tract infections, has been linked to liver irritation in some people too. Put them together and you may stack one liver burden on top of another.

To be fair, this is a theoretical worry based on how each one behaves, not on cases of people taking them together. Still, watch for warning signs like yellowing of your skin or eyes, dark urine, belly pain, or unusual tiredness. Honestly, epistane isn't something I'd recommend at all. Please talk with your doctor or pharmacist before combining these.
