Sitaxsentan sodium falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the main concern when you pair these two: both can be hard on your liver. Sitaxsentan (Thelin) is an endothelin receptor blocker that was used for pulmonary arterial hypertension, and drugs in this class are well known for raising liver enzymes and, in some cases, causing real liver injury. Epistane is a methylated anabolic steroid that is also liver-toxic. Stack them together and you may be doubling up on that stress.

The worry is theoretical, based on how both behave rather than reported cases of this exact combo, but it's a sensible one. Watch for signs like yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Please don't change anything on your own and loop in your doctor or pharmacist first.
