Theoretically, using green tea with diuretic drugs might increase the risk of hypokalemia.
Green tea contains caffeine. In excessive amounts, caffeine can reduce potassium levels due to stimulation of the sodium-potassium pump. Diuretics can also cause lower potassium levels.

Here's the thing to know about Hyzaar: it's a combo pill with two parts. The hydrochlorothiazide (the water pill side) can gradually push your potassium levels down. The losartan side actually tends to nudge potassium up a bit, so the two often balance each other out. Green tea enters the picture mainly through its caffeine. In large amounts, caffeine can lower potassium a little too, which in theory could add to the water pill's effect.

Honestly, this is a theoretical concern based on how these things work, not strong real-world evidence. A few normal cups of green tea are very unlikely to cause trouble. If you're using concentrated green tea extracts or drinking a lot, or you ever feel muscle cramps, weakness, or an odd heartbeat, check in with your pharmacist or doctor.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

If you take Hyzaar (losartan plus hydrochlorothiazide) and enjoy green tea, here's the good news: this is a very minor concern. The interaction on file is based only on lab and animal studies, where green tea showed mixed effects on a liver enzyme called CYP3A4. In actual people, green tea doesn't seem to meaningfully change how this enzyme works, so your blood pressure pills should keep doing their job.

It's worth knowing that losartan is partly activated into its working form by this enzyme system, so in theory blocking it could blunt the effect a bit. But realistically, drinking green tea normally isn't likely to change your blood pressure control, dizziness, or urination. If you use concentrated green tea extract supplements, just mention it to your pharmacist.
