Levofloxacin (ophthalmic) falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Green Tea interacts with.

Theoretically, green tea might reduce the absorption of organic anion-transporting polypeptide (OATP) substrates.
OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds. Research shows that two of the major catechins found in green tea, epicatechin gallate (ECG) and epigallocatechin gallate (EGCG), inhibit OATPs, specifically OATP1A2, OATP1B1, and OATP2B1. In addition, green tea has been shown to reduce the absorption of some drugs that are OATP substrates, including lisinopril, and celiprolol.

Here's the good news: you're asking about the eye drop form of levofloxacin. The concern on file is that green tea catechins (like EGCG) can block gut transporters called OATPs and slightly lower how much of certain swallowed drugs get absorbed. But eye drops work right where you place them, on the surface of your eye, and only a tiny amount ever reaches your bloodstream through your digestive tract. So drinking green tea, or even taking a green tea supplement, isn't going to meaningfully change how your levofloxacin drops fight your eye infection.

On top of that, this interaction is only theoretical, based on lab and animal studies rather than real patients. I wouldn't lose sleep over it here. Still, if you have questions, your pharmacist is happy to double-check with you.

Here's the good news: the interaction on file is about oral levofloxacin, which can slow down how your body clears caffeine (it blocks an enzyme called CYP1A2). That could let caffeine from green tea build up a bit, leaving you jittery, restless, or with a racing heart and trouble sleeping. But you asked about the eye drop version.

With levofloxacin ophthalmic drops, only a tiny amount reaches your bloodstream, so it's very unlikely to affect how you handle caffeine. This concern is really tied to the pills, and it's theoretical even then. Enjoy your green tea, and if you're ever prescribed oral levofloxacin, just mention your caffeine or green tea habit to your pharmacist.

What Is Levofloxacin Used For?

Brand name: Levofloxacin

Levofloxacin treats bacterial infections in adults, including pneumonia (both community-acquired and hospital-acquired), skin infections, urinary tract infections, kidney infections, chronic bacterial prostatitis, and sinus infections. It is also used after exposure to inhalational anthrax and for plague.

Because levofloxacin carries significant risks, it should only be used for uncomplicated urinary tract infections, acute bacterial worsening of chronic bronchitis, and acute bacterial sinusitis when no other treatment option is available. Always use it only for infections confirmed or strongly suspected to be bacterial.
