Hydrochlorothiazide, Losartan Potassium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the honest scoop on combining Gymnema with Hyzaar (that's hydrochlorothiazide paired with losartan). On paper, gymnema has been looked at for slightly slowing down a liver enzyme called CYP3A4, and losartan does pass through that pathway. If that slowing were real, it could theoretically let a little more losartan build up, which might nudge your blood pressure lower than expected (think occasional dizziness or lightheadedness).
But here's the reassuring part: this concern comes only from a single rat-tissue study. Other lab work using human tissue and live animals showed no meaningful effect on that enzyme. So this is a very weak, mostly theoretical worry. The bigger thing to watch is that gymnema may lower blood sugar on its own. Just give your pharmacist or doctor a heads-up before starting it.
