Amiloride, Hydrochlorothiazide falls under POTASSIUM-SPARING DIURETICS — the group Magnesium interacts with.

Potassium-sparing diuretics decrease excretion of magnesium, possibly increasing magnesium levels.
Potassium-sparing diuretics also have magnesium-sparing properties, which can counteract the magnesium losses associated with loop and thiazide diuretics. Theoretically, increased magnesium levels could result from concomitant use of potassium-sparing diuretics and magnesium supplements.

Here's the wrinkle with this particular pill: it's a combo of two water pills. The hydrochlorothiazide side actually tends to flush out magnesium, but the amiloride side is the part that matters here. Amiloride is potassium-sparing, and it holds onto magnesium too. So if you add a magnesium supplement on top, your body may hang onto more of it than usual, and levels could creep up.

Honestly, this is mostly a theoretical concern based on how the drugs work, not something seen often in healthy people. The real risk shows up if your kidneys aren't working well, since that's when extra magnesium can build to the point of causing nausea, flushing, weakness, or a slow heartbeat. Just give your pharmacist or doctor a heads-up before starting magnesium, especially if you have kidney trouble.

About Amiloride and Hydrochlorothiazide

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
