Aspirin, Caffeine, Dover's Pwd falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP1A2. Also, other CYP1A2 substrates might decrease the metabolism of melatonin, increasing melatonin levels.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the piece that matters for this combo: the caffeine in your Acedoval is cleared by a liver enzyme called CYP1A2, and melatonin happens to lean on that same enzyme. On paper, taking them together could slow how fast caffeine leaves your body, which might nudge its effects a little higher (think jitteriness, a racing or fluttery heartbeat, or trouble falling asleep). It can run the other way too, with the caffeine slowing melatonin's breakdown so the melatonin feels stronger.

I want to be honest, though, this is a theoretical interaction based on how these compounds behave, not something proven in real patients. There's also a funny irony: caffeine fights the very sleepiness melatonin is meant to bring. If you take this product regularly, just give your pharmacist or doctor a heads-up before adding melatonin.

Theoretically, melatonin may have anticoagulant effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There are isolated case reports of minor bleeding and decreased prothrombin activity in people taking melatonin with warfarin (Coumadin). The mechanism, if any, of this interaction is unknown. Taking melatonin orally seems to decrease coagulation activity within one hour of dosing in healthy men.

The product you're asking about contains aspirin, which thins the blood a bit by making platelets less sticky. That's where the concern with melatonin comes in. There's a theory, backed by some early lab work and a few scattered case reports, that melatonin can slightly lower the blood's clotting activity on its own. Stack that on top of aspirin and, in theory, you could nudge up the chance of bruising or bleeding more easily.

I want to be honest that this is mostly theoretical and the real-world evidence is thin, so for most folks taking short-term melatonin it's unlikely to cause trouble. Still, keep an eye out for things like easy bruising, nosebleeds, bleeding gums, or dark stools, and check with your pharmacist or doctor before combining them, especially if you take aspirin daily.

Theoretically, taking caffeine with melatonin might increase levels of melatonin.
Some evidence suggests that caffeine consumption can decrease endogenous melatonin levels, while other evidence suggests that caffeine increases endogenous melatonin levels. When administered in combination with melatonin supplements, caffeine seems to increase melatonin effects and levels. The reason for this discrepancy is not completely clear. Part of the discrepancy may result from the fact that caffeine can inhibit melatonin synthesis as well as inhibit melatonin metabolism. By functioning as an adenosine receptor antagonist, caffeine may indirectly inhibit the synthesis of melatonin. Conversely, because melatonin and caffeine are both metabolized by cytochrome P450 1A2 (CYP1A2) enzyme, concomitant use of melatonin and caffeine may reduce the metabolism of melatonin, resulting in higher serum levels.

Here's the thing to know about this combo product: the part that matters for melatonin is the caffeine in it. Both caffeine and melatonin get processed by the same liver enzyme (CYP1A2), so when you take them together, the caffeine can slow melatonin's breakdown and let melatonin levels run a little higher. In practice that usually just means melatonin's calming, sleepy effect may feel a bit stronger.

That said, the caffeine here can also keep you alert and wired, which somewhat works against melatonin at bedtime. The evidence for this interaction is fairly modest, and for most people it isn't dangerous. If you're using this product for pain or a cold and also taking melatonin to sleep, just mention it to your pharmacist so the timing makes sense for you.

This combination product mixes aspirin, caffeine, and Dover's powder (which contains ipecac and opium for pain and cough). The concern on file is mostly theoretical: in lab dishes, some batches of melatonin slowed down an enzyme called CYP3A4. If that happened in your body, drugs cleared by that enzyme could build up a little, leading to slightly stronger effects.

Honestly, I wouldn't lose sleep over this one. The evidence is weak (test-tube only), and none of these ingredients leans heavily on CYP3A4. At most you might notice a bit more drowsiness, since both melatonin and the opium piece can make you sleepy. Still, with an opioid-containing product, it's worth a quick check with your pharmacist before pairing it with melatonin.
