Aspirin, Caffeine, Orphenadrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the piece that matters most in this combo product: the caffeine. Caffeine is cleared by the same liver enzyme (CYP1A2) that helps break down melatonin, so the two can get in each other's way. In theory, melatonin may slow caffeine's breakdown a little, leaving slightly more caffeine in your system. Ironically, that can work against the sleepiness you took melatonin for, and you might notice jitteriness, a faster heartbeat, or trouble falling asleep.
Honestly, this is a theoretical concern based on how these substances are processed, not strong real-world data, so most people won't notice much. The aspirin and orphenadrine parts aren't the issue here. Still, if you take this regularly and rely on melatonin for sleep, mention it to your pharmacist or doctor before combining them.

ANTICOAGULANT/ANTIPLATELET DRUGS

There are isolated case reports of minor bleeding and decreased prothrombin activity in people taking melatonin with warfarin (Coumadin). The mechanism, if any, of this interaction is unknown. Taking melatonin orally seems to decrease coagulation activity within one hour of dosing in healthy men.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the practical piece for this combo product: the part that matters for melatonin is the aspirin in it. Aspirin already thins the blood by making your platelets less sticky, and melatonin may add a small, theoretical anticoagulant effect of its own. Put them together and you could, in theory, nudge your bleeding risk up a bit, meaning things like easier bruising, a nosebleed that takes longer to stop, or stomach upset from the aspirin.
I want to be honest about the evidence though. This is mostly theoretical, with only a few scattered case reports (and those involved warfarin, not aspirin). For most people taking occasional melatonin, it's not a big worry. Still, if you notice unusual bruising or bleeding, or you take this product daily, check in with your pharmacist or doctor first.

CAFFEINE

Some evidence suggests that caffeine consumption can decrease endogenous melatonin levels, while other evidence suggests that caffeine increases endogenous melatonin levels. When administered in combination with melatonin supplements, caffeine seems to increase melatonin effects and levels. The reason for this discrepancy is not completely clear. Part of the discrepancy may result from the fact that caffeine can inhibit melatonin synthesis as well as inhibit melatonin metabolism. By functioning as an adenosine receptor antagonist, caffeine may indirectly inhibit the synthesis of melatonin. Conversely, because melatonin and caffeine are both metabolized by cytochrome P450 1A2 (CYP1A2) enzyme, concomitant use of melatonin and caffeine may reduce the metabolism of melatonin, resulting in higher serum levels.

Here's the quick version: this combo product contains caffeine, and that's the piece that interacts with melatonin. Caffeine and melatonin are both handled by the same liver enzyme (CYP1A2). When you take them together, the caffeine can slow down how fast your body clears melatonin, which may nudge your melatonin levels a bit higher and make its sleepy, wind-down effect a little stronger.
In real life this is usually mild. You might just feel a touch drowsier than melatonin alone would cause. The evidence here is modest, so I wouldn't lose sleep over it (no pun intended). The aspirin and orphenadrine parts aren't the concern. If you take this medicine regularly and want to add melatonin, just run it by your pharmacist or doctor first.

Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist with this combo pill (which contains aspirin, caffeine, and the muscle relaxer orphenadrine). Lab studies suggest melatonin might slow down a liver enzyme called CYP3A4, and orphenadrine is partly handled by that pathway. In theory, that could let a little more orphenadrine linger in your system, nudging up its usual side effects like drowsiness, dry mouth, blurry vision, or feeling lightheaded.
I'll be honest though: this is graded as a minor, theoretical interaction based only on test-tube research, not real patients, so for most people it isn't a big worry. Just keep in mind that both melatonin and orphenadrine can make you sleepy, so the bigger practical issue may simply be extra grogginess. If you take these together regularly, check with your pharmacist or doctor first.
