Abrocitinib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for Cibinqo (abrocitinib): your body partly clears this medicine using a liver enzyme called CYP2C9. In theory, milk thistle (silymarin) might slow that enzyme down a bit, which could let abrocitinib build up a little higher than expected. If that happened, you might notice more of its usual side effects, like nausea, headache, or a higher chance of infections, since this drug already quiets the immune system.

The honest truth is the evidence here is shaky and conflicting. Some lab studies hint at a slowdown, but better human studies suggest milk thistle doesn't meaningfully block CYP2C9. So a real-world problem is unlikely. Still, it's worth a quick mention to your pharmacist or doctor before adding the supplement.

About Abrocitinib

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is used to treat moderate-to-severe atopic dermatitis (eczema) in adults and children 12 years of age and older. It is specifically for people whose eczema has not been adequately controlled with other systemic (whole-body) treatments, including biologics, or when those treatments are not a safe option.

Cibinqo is not recommended to be used at the same time as other JAK inhibitors, biologic immunomodulators, or other immunosuppressants (medicines that reduce immune system activity).
