Acetaminophen, Brompheniramine, Phenylpropanolamine falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

This combo product (Dimetapp Cold and Flu) pairs acetaminophen (the pain and fever reducer) with an antihistamine and a decongestant. The piece that matters here is acetaminophen, since your liver clears a good chunk of it through a process called glucuronidation. Milk thistle, in test-tube studies, seems to slow down the enzyme (UGT) that runs that process.

On paper, that could let a little more acetaminophen hang around. The honest truth is this is theoretical, based only on lab work, so I would not lose sleep over an occasional dose. The bigger real-world concern is simply not stacking too much acetaminophen from multiple cold products, since that is hard on the liver. If you take milk thistle regularly or have any liver issues, run it by your pharmacist first.
