Acetaminophen, Ibuprofen falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Combogesic combines two pain relievers, acetaminophen and ibuprofen. Your liver clears part of the acetaminophen by attaching a sugar molecule to it, a process called glucuronidation. In test-tube studies, the silymarin in milk thistle can slow down the enzyme (UGT) that does this. In theory, that could let acetaminophen build up a little higher than usual.

Here's the honest part: this is based only on lab and animal research, not real people, so the real-world risk looks small. Neither of these drugs is a prodrug, so we're talking about modestly stronger effects, not a loss of pain relief. Still, since acetaminophen is hard on the liver in large amounts, it's worth a quick chat with your pharmacist, especially if you use milk thistle for liver concerns.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Combogesic combines acetaminophen and ibuprofen. The ibuprofen part is partly cleared by a liver enzyme called CYP2C9. The thinking here is that milk thistle might slow that enzyme down, which could let ibuprofen build up a little and, in theory, nudge up its usual side effects like stomach upset, heartburn, or in rare cases kidney strain or higher blood pressure.

Honestly though, the evidence is shaky and the labs disagree with each other. Some research shows no real effect at all, so this is rated minor and unlikely. For most people taking these together short-term, it's not something to lose sleep over. Still, if you use milk thistle regularly or take Combogesic often, mention it to your pharmacist or doctor so they can keep an eye out.
