Amitriptyline, Chlordiazepoxide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

If you're taking Limbitrol (the amitriptyline plus chlordiazepoxide combo) and thinking about milk thistle, here's the honest picture. There's a theory that milk thistle might slow down a liver enzyme called CYP2C9, which helps clear medicines like these. If that happened, the drugs could build up a bit and you might notice more of their usual effects, like drowsiness, dry mouth, constipation, blurry vision, or feeling extra groggy.
The reality is the evidence here is shaky and conflicting. Some lab studies hint at a slowdown, but better human studies suggest milk thistle doesn't meaningfully change CYP2C9. So this is rated minor and unlikely. Still, since these are sedating medicines, run any new supplement by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Limbitrol DS combines amitriptyline (an older antidepressant) with chlordiazepoxide (a benzodiazepine for anxiety). Both are partly broken down by a liver enzyme called CYP3A4. The thinking here is that if milk thistle slows that enzyme down, these medicines could linger a bit longer and feel stronger. With this combo, that might mean more drowsiness, dizziness, dry mouth, constipation, or feeling extra groggy in the morning.
The good news: this is a very weak and uncertain concern. Human studies actually showed milk thistle did not meaningfully change how the body clears CYP3A4 drugs, and the lab data is mixed. I wouldn't lose sleep over it, but since both of these medicines are sedating, it's worth a quick word with your pharmacist or doctor before adding milk thistle.
