Why this applies to Avatrombopag Maleate

Avatrombopag Maleate falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist: your body uses a little cellular pump called P-glycoprotein to help move avatrombopag (Doptelet) around. Milk thistle might slow that pump down a bit, and in theory that could let slightly more avatrombopag get absorbed. Since this drug boosts your platelet count, more of it in your system could, in principle, push platelets higher than intended.

The good news is that this is mostly a theoretical concern. The real-world evidence is weak, and a careful study of a similar pump-dependent drug (digoxin) showed milk thistle didn't actually change its absorption. So a meaningful problem is unlikely. Still, since you're taking Doptelet for a real platelet issue, it's worth a quick chat with your pharmacist or doctor before adding milk thistle, so they can keep an eye on your platelet levels.

About Avatrombopag

What Is Avatrombopag Used For?

Brand name: Doptelet

Avatrombopag treats thrombocytopenia (a low platelet count that raises bleeding risk) in three groups of patients. Doptelet tablets are used in adults with chronic liver disease who need their platelet count raised before a medical procedure, and in adults with chronic immune thrombocytopenia (ITP) whose platelet count has not responded well enough to earlier treatments. Doptelet tablets are also used in children aged 6 years and older with persistent or chronic ITP who have not responded to prior treatment.

Doptelet Sprinkle (oral granules) is specifically indicated for children aged 1 year to less than 6 years with persistent or chronic ITP who have not responded adequately to a previous treatment.
