Bellafoline, Phenobarbital falls under CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might inhibit CYP2B6.
An in vitro study shows that silybin, a constituent of milk thistle, binds to and noncompetitively inhibits CYP2B6. Additionally, silybin might downregulate the expression of CYP2B6 by decreasing mRNA and protein levels.

This combo product pairs belladonna alkaloids with phenobarbital, a barbiturate that calms the nervous system. Phenobarbital is partly broken down by a liver enzyme called CYP2B6. In a lab (test tube) study, a milk thistle compound called silybin appeared to slow this enzyme down. If that happened in the body, phenobarbital could clear a bit more slowly and build up, which might mean more drowsiness, dizziness, confusion, or unsteady walking.

Here is the important part: this is only theoretical, based on lab data, not real patient studies, so I would not panic. Phenobarbital is a sensitive medicine where small changes can matter, though. Please don't start or stop anything on your own. Just check with your pharmacist or doctor first so they can watch for extra sleepiness.
