Why this applies to Celecoxib

Celecoxib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist: your body clears celecoxib mainly through a liver enzyme called CYP2C9. There's some thought that milk thistle might slow that enzyme down a little. If it did, celecoxib could hang around longer and build up, which in theory could nudge up the usual celecoxib side effects like stomach upset, fluid retention, higher blood pressure, or kidney strain.

The honest truth, though, is that the evidence here is shaky and conflicting. Some lab studies hint at a slowdown, but better human research suggests milk thistle doesn't meaningfully affect CYP2C9. This is rated a minor and unlikely interaction. I wouldn't lose sleep over it, but do mention to your pharmacist that you take both, especially if you notice new swelling or tummy trouble.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Celecoxib

What Is Celecoxib Used For?

Brand names: Celebrex, Elyxyb

Celecoxib capsules and the generic oral suspension treat the signs and symptoms of osteoarthritis, rheumatoid arthritis, ankylosing spondylitis (AS), and juvenile rheumatoid arthritis (JRA) in children 2 years and older. Celecoxib capsules also treat acute pain and primary dysmenorrhea (painful periods) in adults.

ELYXYB (celecoxib oral liquid) is specifically approved for the acute treatment of migraine with or without aura in adults — but it is not used to prevent migraines. VYSCOXA (celecoxib oral suspension) is not indicated for acute pain or primary dysmenorrhea.
